Equip Scale Requirements

Equip scales are shipped to patients if all three of the following conditions have been met:

  1. The patient must be 18 years of age or older
  2. The patient's last weight must be less than 400 lbs
  3. In Maud, the patient must have a shipping address listed (shown below)

If all the criteria have been met and your patient started after August 1st, 2024 then they will receive a scale within the first two weeks of treatment automatically. If your patient started before August 1st, 2024 you may submit an Equip Scale order form for the patient here.
