- To request access to Mode, please submit a ticket to Equip Tech Support
- Include if you need access to PHI data or not
- If you are needing PHI data access, please include the business reason for needing the access
- If you already have access to Mode but are needing to view a report that you do not have access to, please submit a ticket to Equip Tech Support.
- Include the report name you are needing access to
- Include the business reason for needing the access to the report
